Who is Eligible:
· Full time employees
Benefit Defined: The Fund will pay full time employees $60 per day to a maximum of 70 days, for a hospital confinement of the employee ordered by a doctor as a result of an accident of illness, including pregnancy, after a 7 day waiting period. The Fund will pay full time employees $30 per day to a maximum of 70 days for a hospital confinement of the employee’s spouse ordered by a doctor as a result of an accident or illness, including pregnancy, after a 7 day waiting period. The discharge date does not count as a day of confinement. Eligibility is determined as of the first day of the confinement upon which the application is based. The daily benefit reimbursement is payable up to 70 days maximum in any continuous 36 month period. The maximum amount payable during any 36-month period is $4,200 per employee and $2,100 per spouse; the 36-month period is calculated from the first date of hospital confinement upon which any claim is paid. A return to the hospital due to the same cause within 14 days of discharge from the hospital will not require a new waiting period. In all other cases a new 7-day waiting period will be required.
Exclusions: This benefit will not be provided for any period of disability caused as a result of self-inflicted wounds, alcoholic or drug rehabilitation, job related illness or injury covered by workers compensation or any injury or illness commencing while on or as a result of US military duty or training as a member of a U.S. Military Reserve or National Guard unit.
Filing Requirements: Claims must be submitted to the Fund office no later than 12 months from the 1st day of hospital confinement.
Limitations: A person may not receive this benefit for the confinement of a spouse if the spouse is an eligible full time employee.
