LONG TERM DISABILITY BENEFIT


Who is Eligible:
·        
Full time employees


Benefit Defined: The Fund will provide, after an eligible employee has been totally disabled for six (6) continuous months, a monthly payment of $400 (*) while disabled and prevented from working as a result of a non-occupational injury or disability. Eligibility shall be determined as of the 1st day of the commencement of the total disability. Benefit payments may continue for a period of up to two years, after the 6 month waiting period, subject to the following: During the first year of disability  benefits, an eligible employee is considered disabled if the eligible employee is unable to work at his/her own occupation. After the first year of disability benefits, an eligible employee is not considered disabled unless the eligible employee is unable to work in any employment or occupation.  (*) Subject to required withholding taxes. Limitations: No Benefits are not playable for:


·         Any injury or illness that is job-related for which you would be covered by Workers Compensation.  


·         Any injury or illness due to war or an act of war. 

 
·      
Any injury or illness which is covered by No-Fault automobile insurance.

  
·       
Any illness or injury commencing while on active, reserve or national guard duty in the military.  


·         Any mental illness of functional nervous disorder.  


·         Any disability caused by intentionally self-inflicted injuries. 

 
·       
Any disability resulting from drug or alcoholic abuse.  


Filing Requirements: The application must be submitted within twelve (12) months of the 1st day of the commencement of total disability.
