Who is Eligible:
· Full time employees.
· Retirees.
Benefit Defined: The Fund will reimburse the full cost of out of pocket expenses for prescription drugs up to $150 per family per calendar year.
Coordination of Benefits: This benefit must be coordinated with any other plan under which a similar benefit is provided so that the total benefits available will not exceed 100% of the actual charges paid.
Filing Requirements: Claims should be submitted once only, either when the maximum $150 has been met or, if less than $150, after January 1st following the year in which the expenses were incurred, but not later than March 31st. No claims will be honored if postmarked after March 31st.
Limitations: If a husband and wife are both full time eligible employees and/or retirees the fund will pay a maximum of $300 per family in any calendar year however, no more than $150 may be submitted for any one person.
